RND Student-Athlete Participation Form (compulsory for every student-athlete/parent/guardian) This form is to be completed on behalf of a student
who wishes to participate in interschool sport and returned to the teacher-coach
prior to the first practice or tryout. RND Senior Letter Information Form (compulsory for graduating student-athletes wishing to be considered for a RND Senior Letter) Graduating Students-Athletes who wish to be considered
for a Regiopolis-Notre Dame Senior Letter must print and complete this
form. Medical Information to Resume Athletic Activity after Injury (for student-athletes/parents/guardians) This form is to be completed by a medical professional
(e.g. physician, chiropractor, physiotherapist) or parent/guardian
or both. Driving Form -- Consent to Travel to an Event or Tournament (for student-athletes/parents/guardians) This form should be carefully read as it outlines
the necessary information and permission to permit a RND student-athlete
to drive or be driven by a teacher, volunteer, parent or student driver
to a tournament or event outside of Kingston. Student-Athlete, OFSAA Transfer Eligibility Appeal Form (for students who have transferred to RND from another Secondary School) When a student transfers from one secondary school
to another, they automatically lose their eligibility to participate
in Interschool sports for a period of 12 months from the time of transfer.
However, students may appeal their loss of eligibility by filling out
the OFSAA Transfer Appeal Form, and attaching the necessary additional
information as it is mentioned on the form. The Athletic Director
can be of assistance when completing the form, and will forward
the information to the Local Sports Association Transfer Appeal Meeting
which occurs at the beginning of each (fall, winter, spring) sports
season. Information Form to Coach as a Community Volunteer at RND (for community members interested in coaching at RND) The following form is to be used when interviewing/considering a potential coach in any one of the following situations: a) The volunteer is not an administrator or someone with teaching certification CLICK HERE to download a .pdf file that will allow you to accurately print this form. of equipment, site, first aid and eligibility guidelines. |
